June 2012 Gecontamineerde ‘speed’ in omloop
Het Wetenschappelijk Instituut Volksgezondheid meldt dat er gecontamineerde amfetamines (speed) in omloop zijn die naast amfetamine ook het toxische 4-methylamfetamine bevatten. De bron ligt vermoedelijk in gecontamineerde basisproducten (precursoren) uit het buitenland.
Onderzoek bij dieren toont aan dat deze stof het effect van normale amfetamine vermindert, wat gebruikers ertoe zou kunnen aanzetten om een extra dosis van deze drug in te nemen. Amfetamine en 4-methylamfetamine werken ook anders in op ons lichaam, waardoor de combinatie van beide stoffen gevaarlijker is dan het gebruik van zuivere amfetamine. Het belangrijkste symptoom van een dergelijke intoxicatie is extreme oververhitting van het lichaam, met mogelijk fatale afloop. Behandeling door een arts is noodzakelijk.
In België zijn er tot nog toe 5 overlijdens ten gevolge van het gebruik van deze gecontamineerde ‘speed’ gemeld. De spoeddiensten, toxicologische laboratoria, politie en preventiewerkers werden via het Belgische Early Warning System voor Drugs geïnformeerd dat deze stof in omloop is.
 
Du ‘speed’ contaminé en circulation
L’Institut Scientifique de Santé Publique signale que des amphétamines contaminées (speed) sont en circulation. Outre de l’amphétamine, elles contiennent également de la4-méthylamphétamine toxique, qui viendrait probablement de produits de base (précurseurs) contaminés issus de l’étranger.
La recherche sur les animaux démontre que cette substance réduit les effets del’amphétamine normale, ce qui pourrait inciter les consommateurs à en prendre une dose supplémentaire. L’amphétamine et la4-méthylamphétamine ont des mécanismes d’action différents sur l’organisme, rendant la combinaison des deux substances plus dangereuse que la consommation d’amphétamine pure. Le symptôme principal d’une telle intoxication est une surchauffe extrême du corps qui peut s’avérer mortelle.
Une prise en charge par un médecin est nécessaire. En Belgique, jusqu’à présent, il est fait état de 5 décès dus à une consommation de speed contaminé. Les services d’urgence, les laboratoires toxicologiques, la police et les préventionnistes ont été informés de la circulation de cette substance grâce au Early Warning System belge (système d’alerte précoce) pour les drogues.

Het BEWSD werd op de hoogte gebracht van de analyse van verschillende tabletten met een gevaarlijk hoog gehalte aan MDMA.De tabletten zijn afkomstig uit Antwerpen en hebben volgende kenmerken:

A.Blauwe tablet met ‘DANCE’-logo aan één zijde en ‘Defqon - Y’-logo aan de andere zijde (gemiddelde diameter: 8,1 mm; gemiddelde dikte: 5,4 mm; gemiddeld gewicht: 318 mg) (http://www.ecstasydata.org/view.php?id=2596
B. Blauwgrijze tabletten met ‘N°1’-logo (gemiddelde diameter: 8,5 mm; gemiddelde dikte: 4,3 mm; gemiddeld gewicht: 221 mg).
Hooggedoseerde tabletten, vooral als gebruikers hier meerdere tabletten van consumeren, brengen o.a. het risico van oververhitting en een serotonine-syndroom met zich mee.

It seems that recently, a batch of 5-IT has been mislabelled, and was sold on the internet as 5-APB or 6-APB.
Considering the difference in dosage, a lot of users in Sweden thought they were consuming APB, and, consequently, overdosed, with 14 deaths reported in 2012. See also EMCDDA alerts.

Swedish Poisons Information Centre (GIC) report 9 cases with 5-IT during Jan-June 2012. 4 cases together with other drugs. 2 additional cases during July 2012. Symptoms: Dilated pupils, sweating, restless, disorientated, agitated, anxiety, high heart rate, high blood pressure, high body temperature.
One example: Female 18 yrs. has taken 1 capsule 5-IT unknown strength. 15 hours after drug intake restless, agitated, disorientated, shivering, dilated pupils, sweating and a heart rate 160/min. Rising body temp until 38,6 C. Treatment with sedating drugs without sufficient effect . She sedated and put in a respirator. Full of anxiety when awake the next day otherwise in stable condition. GIC cannot analyze 5-IT yet the above information comes from their telephone services. There is a collaboration within the STRIDA project soon resulting in confirmed chemical analysis of some 5-IT cases reported to GIC.

"Latest information concerning 5-IT: The substance has been found in 14 postmortem examinations. In four of the cases no data on quantity are available yet, in the rest (10 cases) the concentration in postmortem blood range from 0,7-5,2 microgram/g blood. In one case 18,6 microgram/g femor blood. All cases are men. 12 cases between 20-30 years and two cases over 30 year. Postmortem examinations were done during first half of April and June 30. In two cases 5-IT is the single substance found but in the other cases medical drugs and other illicit drugs could be found. So far two cases are verified with the cause of death related to 5-IT intoxication."

The National Laboratory of Forensic Toxicology (RMV) has observed the substance 5-IT in 12 postmortem examinations from femur blood. In at least 3 cases the cause of death is considered to be due to intoxication with 5-IT. The rest of cases are still under investigation. In several cases the police found an empty bag labeled 6-APB, which could not be detected in blood. Instead 5-IT was detected. That is all information for the moment, but RMV will put together more information in this matter. Our contact there is Gunilla Thelander.

Dear colleagues,
 
In the past week several powders were analysed by the Dutch drug checking service DIMS that were found to contain the dangerous substance atropine.
The tested powder samples were cocaine or ketamine samples, contaminated with atropine, submitted for testing in the region of Noord-Brabant.
 
At the same time we also received information about at least three cases in the region of Antwerp where atropine was detected in the urine of known cocaine users.
 
Intoxications with atropine are characterised by the following symptoms: restlessness, dilated pupils, dry skin, dry mouth, blurred vision, fever, panic,
palpitations, confusion, hallucinations, decreased consciousness and epilepsy symptoms. Respiratory depression and irregular heartbeats can result in fatal cases.
 
For the moment we only received notifications for the region of Antwerp and The Netherlands; however, it cannot be excluded that these contaminated drugs are present in other regions in Belgium as well.
In Belgium drug checking services are available atModus Fiesta/Modus Vivendi in Brussels.
 
We would like to warn about this potential life-threatening contamination of cocaine and/or ketamine. Please distribute this message in your networks.

​​​​The Belgian Early Warning System Drugs (BEWSD) at the Scientific Institute of Public Health (WIV-ISP) would like to report the identification and analysis of several highly dosed ecstasy tablets. 

Two tablets were reported by the WIV-ISP Medicines laboratory in the framework of the drug testing project at Modus Vivendi. 

Another four tablets were reported by the National Institute for Crimina​listics and Criminology (NICC-INCC).

 
All tablet characteristics can be found in the table below.


We would like to draw your attention specifically to these tablets, and would like to remind you that the majority of ecstasy tablets currently circulating remains high to very highly dosed.

Dosages of up to 300 mg MDMA base per tablet have been reported in the past.



The only reliable method to determine the amount of MDMA present in an ecstasy tablet is by extensive laboratory analysis; color reagents tests such as Marquis never provide dosage indications.

 

Symptoms of an MDMA overdose are characterised by heavy sweating, tremor, dry mouth, nausea, vomiting, elevated blood pressure and heart rate, paranoia and hyperthermia. 

In severe cases a serotonin syndrome can occur, resulting in an often fatal hyperthermia. Overdose treatment is symptomatic.

​Dear colleagues,
 
The Belgian Early Warning System Drugs (BEWSD), the drug monitoring system of the Scientific Institute for Public Health (WIV-ISP) would like to inform you abouta recent death caused by the consumption of 3-MeO-PCP.
 
The death happened in the first half of June in the region of Ghent, and concerns a male, age 35, with no previous history of known drug (ab)use.
A powder was found on the scene which was probably consumed by sniffing.
 
After analysis by the Ghent University Toxicology Laboratory (prof. dr. Stove), the presence of 3-MeO-PCP was identified; metabolites such as phencyclidine were also identified in post-mortem blood and urine.
It is currently unknown whether the victim knowingly consumed this substance, or was intending to consume some other psychoactive substance.
 
3-MeO-PCP is a hallucinogenic dissociative substance that is illegal in Belgium, and belongs to the arylcyclohexylamine class of chemicals. It is a close derivative of the dissociative anesthetic phencyclidine (PCP).
The effects of 3-MeO-PCP are extremely similar to those of PCP and include pronounced dissociation, sedation, and hallucinations, with oral dosages starting at 5 mg.
 
This is the first time a death caused by 3-MeO-PCP was reported in Belgium. 3 deaths associated with this substance were reported in Sweden in 2014, and one death occurred in Portugal in 2016.
Treatment of intoxications with 3-MeO-PCP is mainly supportive and symptomatic, there are no known antidotes.
 
Some remarks:
1. Victims were mostly not known drug (ab)users, but it seems that they were experimenting with substances acquired over the internet.
2. There is some doubt whether the victims consumed the substances they intended to consume: mislabeling was confirmed in at least one case.
As a result, the victims could have consumed (much) larger dosages than intended, resulting in overdose and ultimately, death.

We take this opportunity to remind you that the majority of ecstasy tablets currently circulating are highly dosed, sometimes containing dosages of up to 300 mg MDMA base
(the purity and MDMA dosage in ecstasy tablets can only be determined using laboratory analysis).
 
Symptoms of a MDMA overdose are characterised by heavy sweating, tremor, dry mouth, nausea, vomiting, elevated blood pressure and heart rate, paranoia and hyperthermia. 
In severe cases, a serotonin syndrome can occur, resulting in an - often fatal -hyperthermia.

March 2017 - Detection of highly dosed MDMA tablets, MDA and THJ-2201
 
Dear colleagues,

The Belgian Early Warning System at the Scientific Institute for Public Health (WIV-ISP) was recently notified about the identification of the following substances and/or samples:
 
* a new substance, THJ-2201
* several highly dosed MDMA tablets
* a tablet only containing MDA.
 
These drug samples were collected in the framework of the drug testing project at Modus Vivendi, Brussels.

THJ-2201 is a synthetic cannabinoid, that was first identified in Europe at the end of 2013. It was identified for the first time in Belgium in 2014 (seizure by customs).
The powder sample containing this substance was deposited at Modus Vivendi as being methiopropamine (MPA) and was presumably obtained through on-line purchase.
It is currently unknown to what extent this powder is being used in Belgium, although it seems unlikely this phenomenon will be wide-spread.

A furtherfour tablets were collected and analyzed, and found tocontain more than 150mg of MDMA (expressed as MDMA base).
Tablet characteristics including photographs, can be found in the table below.
The majority of ecstasy tablets currentlycirculating are highly dosed, sometimes containing dosages of up to 300 mg MDMA base.
Symptoms of MDMA overdose are characterized by heavy sweating, tremor, dry mouth, nausea, vomiting, hypertension and tachycardia, paranoia and hyperthermia.
In severe cases a serotonin syndrome can occur, resulting in an often fatal hyperthermia.
 
One collected tablet did not contain MDMA but rather the close derivative MDA, the demethylated derivative (and metabolite) of MDMA.
The psychoactive effects of MDA are similar to those of MDMA, the main difference being that MDA provokes pronounced hallucinogenic effects.
Preliminary animal data could suggest a higher (neuro)toxicity for MDA compared to MDMA.

​​​The Belgian Early Warning System Drugs (BEWSD) at the Scientific Institute of Public Health (WIV-ISP) would like to report the identification and analysis of several highly dosed MDMA tablets. 
These tablets were analysed in the framework of a BEWSD study during several summer festivals between July and end of September.Some tablets were analysed by the National Institute for Criminalistics and Criminology (NICC), and are included as well.
The tablets  shown contain more than 200mg of MDMA base (picture 1). The second picture contains tablets with a MDMA base content of 150 to 200 mg. All further characteristics can also be found in thepictures.
The BEWSD would like to draw your attention specifically to those results. Dear colleagues,
 
The Belgian Early Warning System Drugs at the Scientific Institute of Public Health (WIV-ISP) was informed about the analysis of a blotter containing the new psychoactive substance: 25C-NBOMe, a substituted phenethylamine and derivative of 2C-C.
This substance was not been detected in Belgium so far.
The pink blotter with drawings bought in Liège was submitted through the Modus Fiesta drug testing project in Brussels, assuming to be LSD but containing 25C-NBOME.
It is a potent partial agonist of the serotonin 5-HT2A receptor in particular and appears to have stimulant and particularly hallucinogenic effects. It has been associated with non-fatal intoxication (tachycardia, hypertension, seizures, hyperpyrexia) and death. It has been reportedly sold as LSD or as a ‘legal’ alternative to LSD or “research chemical” usually via Internet websites. The variation in formulations and resultant dosage coupled with its potency results in health risks to the individual.

In addition, the BEWSD has again been informed of the analysis ofan ecstasy tablet containing a high amount of MDMA.

The Belgian Early Warning System Drugs at the Scientific Institute of Public Health (WIV-ISP) was informed on the analysis of a tablet containing the new psychoactive substance: pCPP (para-chlorophenylpiperazine), a synthetic piperazine derivative.
This substance was not been detected in Belgium so far.
The tablet was submitted through the Modus Fiesta drug testing project in Brussels; assuming to be ecstasy but containing pCPP and amphetamine base.

A number of piperazine derivatives has been reported as recreational drugs. The isomers, predominantly mCPP, have been used as surreptitious substitutes for MDMA and have also been found as contaminant in amphetamine products.
Scientific research has demonstrated pCPP to have serotonergic effects, likely acting as a non-selective serotonin receptor agonist and/or releasing agent.
Possible adverse effects areheadaches, nausea, severe hallucinations.

In addition, the BEWSD has again been informed of the analysis ofseveral ecstasy tablets containing a high amount of MDMA.

The European Monitoring Centre forDrugs and Drug Addiction has just released an EWS alert concerning the appearance of the new substance U-47700 in Europe.
This substance has already been detected in Belgium : on January 22, 2016, the Belgian Early Warning System Drugs (BEWSD) at the Scientific Institute of Public Health (WIV-ISP) disseminated an alert concerning a deathafter consumption of fentanyl and U-47700.
 
Considering the relevance and the toxicity of the substance, we would like to inform the BEWSD network of this EMCDDA EWS alert, which can be found in attachment.
 
U-47700 is a new psychoactive substance with strong agonist activity at the mu-opioid receptor. Potency is estimated at about 7-8 times that of morphine.

The Belgian Early Warning System Drugs (BEWSD) at the Scientific Institute of Public Health (WIV-ISP) was informed of the analysis of two ecstasy tablets containing a high amount of MDMA.
These tablets were analysed in the framework of the Modus Fiesta drug testing service in Brussels.
 
The tablet characteristics can be found in the table below, together with pictures of the tablets.

 
 
The BEWSD would like to draw your attention to these tablets, and would like to remind you that the majority of ecstasy tablets currently circulating are highly dosed, sometimes containing dosages of up to 300 mg MDMA base.
The amount of MDMA present in an ecstasy tablet can only be determined using laboratory analysis.
 
Symptoms of an MDMA overdose are characterised by heavy sweating, tremor, dry mouth, nausea, vomiting, elevated blood pressure and heart rate, paranoia and hyperthermia.
In severe cases a serotonin syndrome can occur, resulting in an often fatal hyperthermia.

Geachte collega’s,
 
Het Belgisch Early Warning System Drugs (BEWSD), een service van hetWetenschappelijk Instituut Volksgezondheid (WIV-ISP), werd geïnformeerd over een ecstasytablet die p-methoxyamfetamine (PMMA) bevat ipv MDMA.
Deze tablet werd aangetroffen in de regio Kortrijk, en heeft volgende kenmerken:
 
Logo: “Rolling Stones”
Kleur: grijs/wit
Diameter: 8.1 mm
Dikte: 6 mm
Gewicht: 243.6 mg
Gehalte: 62.1 mg PMMA


Een intoxicatie met PMMA kan aanleiding geven tot extreme hyperthermie, samen met hyperactiviteit, angst, stuipen en rhabdomyolyse.

In hoge doseringen of in combinatie met andere drugs is deze stof dodelijk.
PMMA is wereldwijd verantwoordelijk voor tientallen doden; ook in ons land werden dit jaar overlijdens gemeld na consumptie van PMMA-tabletten (bv. de Superman-tabletten).
 
Bovendien wijst het BEWSD erop dat de grote meerderheid van de ecstasy tabletten op de markt hoog tot zeer hoog gedoseerd zijn. Tabletten met tot 300mg MDMA zijn geen uitzondering.
Afgaande op het uiterlijk kan men nooit zekerheid hebben over de inhoud of dosis van een tablet.
Integendeel, we merken meer en meer dat gekende, hoog gedoseerde tabletten van hoge kwaliteit dikwijls “gekopieerd” worden, waarbij er een andere dosering (of zelfs een totaal andere psychoactieve substantie) in deze namaak-tabletten kan aanwezig zijn.
Twee schijnbaar identieke tabletten kunnen dus een heel verschillende werking of toxiciteit hebben.
De enige manier om zekerheid te hebben over de inhoud van een tablet is door een labo-analyse. In ons land kan dit bv. bij Modus Vivendi te Brussel.

Il est difficile d’identifier les tablettes sur base de leur aspect visuel. Leur contenu ne peut être établit en effet qu’après une analyse de laboratoire. En Belgique il est possible de faire usage du service de testing de produits de Modus Vivendi.

Naar aanleiding van de “red alert” van het Nederlands Trimbos-instituut wil het Wetenschappelijk Instituut Volksgezondheid via zijn Belgian Early Warning System Drugs dit alert ook doorgeven.
Het betreft een ecstasy-tablet met het logo “ADE”, die circuleert en levensgevaarlijk hoge dosissen MDMA bevat.
Deze werd geproduceerd in het kader van het Amsterdam Dance Event van 14-18 oktober.

Tot op heden werden deze tabletten niet aangetroffen in België.
Red alerts in Nederland zijn zeer zeldzaam, engezien de vergelijkbare ecstasy-markt in beide landen, willen wij ook expliciet waarschuwen om deze tablet ten allen kost te vermijden.
 
Het gebruik van hoog gedoseerde ecstasy houdt zware risico’s in met sterk verhoogde kans op overdosering, waardoor oververhitting, acute psychotische verschijnselen, hartritmestoornissen en leverfalen kunnen optreden met mogelijk fatale gevolgen.

L’institut hollande Trimbos, la centre de recherche drogue aux Pays Bas, a distribué un “Red Alert” concernant des tablettes d’ecstasy avec un très grand dosage en MDMA, qui sont potentiellement mortelle.
Il s’agit d’un comprimé d'ecstasy avec le logo «ADE», qui contient des doses extrêmement élevé en MDMA.
Les comprimés ont été produits dans le cadre de l'Amsterdam Dance Event du 14-18 Octobre.

Les « alertes rouges » dans les Pays-Bas sont très rares. Tenant compte du marché de l'ecstasy similaire dans nos deux pays, le BEWSDde l’Institut de Santé Publique veut aussi vous avertir explicitement à éviter cette tablette à tout prix.
 
La consommation d'ecstasy à haute dose augmente le risque de surdosage, provoquant une hyperthermie extrême, des symptômes psychotiques aigus, des arythmies et d'insuffisance hépatique, avec des conséquences potentiellement fatales.

Geachte collega’s,
 
Het Wetenschappelijk Instituut Volksgezondheid (WIV-ISP) en het Belgian Early Warning System Drugs werden op de hoogte gebracht van verschillende ecstasy-tabletten waarin nieuwe psychoactieve stoffen werden aangetroffen.
 
Tevens werden verschillende tabletten geïdentificeerd waarin een hoge tot zeer hoge dosis MDMA werd gevonden.
Een deel van deze tabletten werd geïdentificeerd door de pill test service van Modus Vivendi te Brussel. De overige tabletten werden geanalyseerd in het kader van een BEWSD onderzoeksproject.
 
Het gaat om volgende nieuwe psychoactieve substanties (NPS):
* 4-CA (chlooramfetamine), CMA (4- chloormethamfetamine), PMA, PMMA en ethylone
* DOB (2,5-dimethoxy-4-bromo-amfetamine)
* a-PVP (α-Pyrrolidinovalerophenone)
* 2-CB (2,5-dimethoxy-4-bromophenethylamine)
 
CMA (en zijn derivaat chlooramfetamine) werden al gerapporteerd door het BEWSD.
Dit zijn nieuwe, nog legale stoffen die neurotoxisch zijn voor serotonerge neuronen in de hersenen, en voor blijvende hersenschade kunnen zorgen.
De specifieke gevolgen op korte noch lange termijn zijn bekend. Het is de tweede maal op zeer korte tijd dat tabletten met deze stof worden gevonden in ons land.
PMA en PMMA zijn al langer bekend; het zijn gevaarlijke amfetamine-derivaten die in ons land al voor tientallen doden hebben gezorgd.
Grootste gevaar hierbij is extreme oververhitting, zeker in combinatie met MDMA of bij gebruik van grotere hoeveelheden. Ethylone is een synthetisch cathinone-derivaat, met stimulerende werking.
 
DOB is een hallucinogeen amfetamine-derivaat met een zeer krachtige werking.
Kenmerkend aan deze stof is de extreem lange werkingsduur (24-48u trips zijn geen uitzondering), en de lage werkzame dosis.
Bij gebruik van te grote doseringen is deze stof heel toxisch.
 
a-PVP is een cathinone-derivaat, en wordt vooral gebruikt omwille van zijn stimulerende werking.
Ze is bij het grote publiek bekend onder de naam “Flakka”. Deze stof wordt nog dit jaar toegevoegd aan de lijst met gecontroleerde substanties.
 
2-CB is een “designer drug” uit de jaren ’90. Zij was lange tijd legaal in Nederland, tot eind jaren negentig.
Het is een hallucinogene stof, waarvan het effect enigszins te vergelijken is met het effect van hallucinogene paddenstoelen (“magic mushrooms”).
 
Verder werd ook nog een tablet aangetroffen met diclofenac, een ontstekingsremmend geneesmiddel.
 
De overige tabletten zijn hoog gedoseerde ecstasy tabletten die 2 tot 3 maal de gebruikelijke dosis MDMA bevatten.
Alle gegevens met betrekking tot het uiterlijk en de dosering (indien gekend) zijn samengevat in onderstaande tabel.
 
Het BEWSD wijst erop dat momenteel de grote meerderheid van de ecstasy tabletten op de markt hoog tot zeer hoog gedoseerd zijn.
Tabletten met tot 300mg MDMA zijn geen uitzondering.

Le Belgian Early Warning System Drugs, de l’Institut Scientifique de Santé Publique (WIV-ISP), a été informé de la détection de plusieurs nouvelles substances psychoactives dans des pilules d’ecstasy,ainsi que de pilules hautement dosées en MDMA.
Un de ces comprimés a été détecté par Modus Vivendi à Bruxelles, alors que les autres ont été analysés dans le cadre de recherche réalisées par le BEWSD et ses partenaires.
 
Il s’agit denouvelles substances suivantes :
* 4-CA (chloro-amphétamine), CMA (4- chloro-méthamphétamine), PMA, PMMA en ethylone
* DOB (2,5-dimethoxy-4-bromo- amphétamine)
* a-PVP (α-Pyrrolidinovalerophenone)
* 2-CB (2,5-dimethoxy-4-bromophenethylamine)
 
Le CMA a déjà fait l’objet d’une alerte il y a deux semaines. C’est un dérivé de la méthamphétamine pour lequel il existe peu d’information.
Actuellement cette substance est légale. Le 4-CA, une substance dérivée, était présente aussi.
Au départ, le CMA a été étudié pour ses propriétés antidépressives, mais les recherches ont été arrêtées après la découverte de son action neurotoxique.
Cette substance est extrêmement toxique et est susceptible de causer des dommages durables au système neurotransmetteur à la sérotonine.
 
La PMA et la PMMA sont des substances connues depuis longtemps. Ce sont des dérivés d’amphétamine qui ont causé des dizaines des morts dans notre pays.
L’hyperthermie grave est le plus grand danger, particulièrement en cas de combinaison avec la MDMA. L’ethylone est une cathinone de synthèse avec des effets stimulants.
 
Le DOB est un hallucinogène, dérivé d’amphétamine, qui est très puissant.
La durée de l’effet est extrêmement long : entre 24 et 48h. La dose active est basse (quelques milligrammes); le DOB peut donc être toxique en cas de dosage élevé.
 
a-PVP est une cathinone de synthèse avec des effets stimulants, mieux connue comme « Flakka ».
La substance sera ajoutée à la liste des substances contrôlées à la fin de cette année.
 
La 2C-B est un « designer drug » apparu dans les années 90. Elle est restée légale au Pays-Bas jusqu’en 1996.
C’est un hallucinogène, avec un effet « doux », comparable aux champignons hallucinogènes.
 
On a aussi trouvé un comprimé contenant du Diclofenac, un médicament anti-inflammatoire.
 
Les autres tablettes sont des tablettes d’ecstasy hautement dosées en MDMA, contentant 2-3 fois plus qu’un dosage « normal ».
 
Toutes les caractéristiques et les photos se trouvent dans le tableau ci-dessous.
 
Comme un grand nombre de tablettes d’ecstasy circulent actuellement avec un dosage élevé en MDMA (même jusqu’au 300mg), nous conseillons d’être extrêmement prudent.

Het Belgisch Early Warning System Drugs op het Wetenschappelijk Instituut Volksgezondheid (WIV-ISP) werd op de hoogte gebracht van de ontdekking van de nieuwe psychoactieve substantie4-chloromethamphetamine (CMA).
Deze stof werd eind juli aangetroffen in een ecstasy-tablet, in de regio Antwerpen.
 
CMA is een derivaat van methamfetamine waarover zeer weinig bekend is. Het is de eerste maal ooit dat deze stof wordt aangetroffen, op dit ogenblik is deze stof nog legaal.
Oorspronkelijk werd CMA onderzocht als antidepressivum, maar na ontdekking van de neurotoxische eigenschappen werd dit onderzoek stopgezet.
De stof is bijzonder toxisch en veroorzaakt blijvende schade aan het serotonerge neurotransmittersysteem.
Daarnaast is ze ook acuut gevaarlijk: zeker in combinatie met bv. MDMA bestaat een risico op een serotonine syndroom en extreme hyperthermie, met mogelijk de dood tot gevolg.
Tot op heden werden geen intoxicaties of overlijdens gemeld gerelateerd aan CMA.
 
CMA werd in ons land aangetroffen in een rechthoekige gele tablet met logo “Durex”.

Graag willen wij u vragen om alle beschikbare informatie omtrent deze tabletten te melden aan het BEWSD.
Gezien momenteel veel ecstasytabletten circuleren met een hoge dosering aan MDMA raden we aan bijzonder voorzichtig te zijn.
 
Op dit moment is de “Durex” tablet de enige gekende tablet waarin CMA werd aangetroffen.
Het kan echter niet uitgesloten worden dat CMA ook aanwezig is in andere ecstasy-tabletten. Dit kan echter enkel vastgesteld worden na een laboratoriumanalyse.
In België kan men hiervoor gebruik maken van de pill test service van Modus Vivendi.
 
Het BEWSD volgt de situatie van nabij op, en zal u op de hoogte brengen van nieuwe ontwikkelingen.Het persbericht van het WIV-ISP kan uhier vinden.

Le Belgian Early Warning System Drugs, de l’Institut Scientifique de Santé Publique (WIV-ISP), a été informé de la présence de la nouvelle substance psychoactive4-chloromethamphétamine (CMA).
Cette substance a été découverte fin juillet dans une tablette d’ecstasy dans la région d’Anvers.
 
Le CMA est un dérivé de la méthamphétamine pour lequel il existe peu d’information. C’est la toute première fois que cette substance est rapportée. Actuellement cette substance est légale.
A départ, le CMA a été étudié pour ses propriétés antidépresseur, mais après la découverte de propriétés neurotoxiques, les recherches ont été stoppées.
La substance est extrêmement toxique et cause des dommages durables au système neurotransmetteur de sérotonine.
De plus il est également dangereux de manière immédiate : certainement en combinaison avec par exemple la MDMA il existe un risque d’un syndrome sérotoninergique et d’une hyperthermie extrême avec la possibilité de conséquences létales.
Jusqu’à présent il n’y a eu aucune intoxication ou décès liés au CMA qui ont été rapportés.
 
Le CMA a été découvert chez nous sous la forme d’une tablette rectangulaire avec le logo « Durex » :

Nous souhaiterions vous demander de signaler au BEWSD toute information disponible concernant ces tablettes.
Comme un grand nombre de tablettes d’ecstasy circulent actuellement avec un dosage élevé en MDMA, nous conseillons d’être extrêmement prudents.
 
Actuellement la tablette “Durex” est la seule tablette connue contenant du CMA. Il ne peut pas être exclu que du CMA soit également présent dans d’autres tablettes d’ecstasy.
Ceci ne peut être établit qu’après une analyse de laboratoire. En Belgique il est possible de faire usage du service de testage de pilules de Modus Vivendi.

Geachte collega’s,





Het Belgian Early Warning System Drugs van het Wetenschappelijk Instituut Volksgezondheid (WIV-ISP) werd op de hoogte gebracht vanverschillende ecstasy-tabletten waarin geen actieve stof werd aangetroffen.





Deze tabletten werden in juli aa​​​​ngetroffen in de omgeving Antwerpen, met de logo’s “granaat”, “gold” en “UPS”, en bevatten verder geen actieve of toxische stoffen (enkel bv. lactose).



Het BEWSD wil opmerken dat tabletten met identiek uiterlijk bij het Nederlandse DIMS bekend staan als hoog gedoseerd aan MDMA. De kans bestaat dat deze hoog gedoseerde tabletten ook bij ons op de markt zijn.



Gezien de tabletten qua uitzicht identiek zijn, is het niet mogelijk te bepalen welke tabletten MDMA bevatten en welke niet.



Het BEWSD raadt dan ook sterk aan deze tabletten te vermijden.



 



Tijdens de pill test service van Modus Vivendi werd eveneens een hoog gedoseerde MDMA tablet aangetroffen. Deze tablet werd aangekocht in de regio Brussel, en heeft het logo “Rolls Royce”.



De tablet bevat meer dan het dubbele van een standaarddosis MDMA. Alle eigenschappen van deze tabletten zijn te vinden in onderstaande tabel.





Momenteel zijn er zeer veel ecstasy tabletten op de markt met een hoge tot zeer hoge dosis MDMA. Tabletten met 250-300mg MDMA zijn geen uitzondering meer.




We merken zelfs dat gekende, hoog gedoseerde tabletten regelmatig “gekopieerd” worden, waarbij er een andere dosering (of zelfs helemaal geen actieve stof, zoals in dit bericht) kan aanwezig zijn.



Twee schijnbaar identieke tabletten kunnen dus een heel verschillende werking of toxiciteit hebben.



Enkel een laboratoriumanalyse kan zekerheid bieden omtrent de inhoud of dosering van een ecstasy tablet.



Het testen van tabletten is in ons land mogelijk in Brussel, bij Modus Vivendi.

Chers collègues,



Le Belgian Early Warning System Drugs à l’Institut Scientifique de Santé Publique (WIV-ISP) a été informé au sujet de différentes tablettes d’ecstasy ne contenant aucune substance psychoactive.




Ces tablettes ont été découvertes en juillet dans la région d’Anvers et elles ont des logos « grenade », « Gold » et « UPS ». Le seul contenu était du lactose.



Le BEWSD veut attirer l’attention sur le fait que des tablettes avec une apparence identique sont connues aux Pays-Bas par le DIMS comme tablettes hautement dosées en MDMA.



Ces tablettes hautement dosées peuvent être également disponible sur le marché belge.



Comme il est impossible pour un utilisateur de faire la distinction sans analyse chimique entre ces tablettes, le BEWSD conseille dès lors d’en éviter la consommation.



 



Aussi, le service de pill testing de Modus Vivendi a découvert une tablette contenant du MDMA hautement dosé (logo Rolls Royce, voir tableau). Cette tablette a été achetée dans la région de Bruxelles.



Le dosage en MDMA est plus que le double d’un dosage standard. Toutes les données concernant l’aspect et le dosage sont résumées dans le tableau ci-dessous.




Actuellement la majorité des tablettes d’ecstasy sur le marché sont hautement dosées. Des tablettes contenant jusque 250-300mg de MDMA ne sont plus une exception.




Sur base des caractéristiques extérieures d’une tablette, il n’est jamais possible d’avoir une certitude sur le contenu ou la dose de celle-ci.



Au contraire, nous remarquons de plus en plus que des tablettes connues, hautement dosées de bonne qualité sont souvent copiées dans lesquelles on peut retrouver un autre dosage



(ou même une autre substance psychoactive, ou rien du tout). Deux tablettes visiblement identiques peuvent donc avoir un effet et une toxicité très différente.



La seule manière d’être certain du contenu d’une tablette est grâce aux analyses de laboratoire. Dans notre pays, cela peut se faire par exemple chez Modus Vivendi à Bruxelles.

The Belgian Early Warning System Drugs (BEWSD) was informed of the analysis of several ecstasy tablets, containing dangerously high amounts of MDMA, the active substance in ecstasy-tablets.
These tablets were analysed after a user submitted them to the Modus Vivendi pilltesting project in Brussels.
 
It concerns tablets with following characteristics:

Wound botulism in people who inject heroin in Norway and the United Kingdom
 ECDC/EMCDDA, Lisbon, February 2015

The European Centre for Disease Prevention and Control (ECDC) and the EMCDDA released a joint risk assessment in February 2015,
on wound botulism among people who inject heroin in Norway and the United Kingdom.
 
At the time of publication, a total of 23 cases of wound botulism had been reported in the two countries.
The source of infection was thought to be a batch of contaminated heroin, no cases have been found in Belgium.

The Belgian Early Warning System on Drugs (BEWSD) of the Scientific Institute of Public Health (WIV-ISP) would like to warn for the following new psychoactive substances, recently found in Belgium.
 
Several blotter papers containing 25I-NBOMe and 25C-NBOMe were found in September in the region of Liège.

We strongly advize to avoid these blotters.
The action of 25I-NBOMe and 25C-NBOMe can be compared with the action of other hallucinogens such as LSD.
Contrary to LSD, several intoxications and deaths have already been reported internationally for 25X-NBOMe-derivatives.
Treatment of intoxications with 25I-NBOMe are mainly symptomatic, a specific antidote is not available.
In severe cases aggressive sedation with intubation might be necessary.
 
Through Modus Vivendi, several powder samples were analysed containing different new psychoactive substances. 
The following substances were found:
 
* AB-FUBINACA, found in a beige powder
* AM2201 and MT-45, found combined in a white powder
* 2C-E, found in a white powder.
 
AB-FUBINACA and AM-2201 are synthetic cannabinoids. These substances have a similar psychoactive effect as natural cannabis, and are most frequently used as a replacement for herbal cannabis, for example in persons that undergo regular urine testing. Very little is known about this compounds with regards to toxicity. Dosages are very low (<1 mg). Internationally, several intoxications and even deaths have occurred after the use of these substances.
AM-2201 has already been found in Belgium in the past.
 
MT-45 is a synthetic opioid. Users report effects which are comparable to the effects of codeine.
Intoxications with MT-45 are characterised by typical opioid overdose symptoms such as respiratory depression, loss of consciousness, decreased heart rate, urinary retention, severe miosis.
The specific antidote for opioid poisonings, naloxone, should be administered immediately in case of an overdose.
 
Since both AM-2201 and MT-45 are depressants, the combination of both in the same powder can be especially dangerous.
 
2C-E is a hallucinogenic amphetamine derivative bearing a close resemblance to the popular (and illegal substance) 2C-B.
Toxicology and effects of 2C-E can be compared to those of 2C-B, although users report the effects of the drug to be “less desirable”.
 
The BEWSD would like to warn for the presence of these substances in Belgium.
We would also like to ask you to inform us should you encounter these substances.

The Belgian Early Warning System Drugs (BEWSD) was informed of the analysis of several ecstasy tablets, containing either very high quantities of MDMA or the new psychoactive substance 4-fluoro-amphetamine (4-FA/4-FMP).
These tablets were seized during the summer of 2014.
 
Considering that at least one person died this year after the consumption of a highly dosed MDMA tablet, the BEWSD would like to warn for the presence of these tablets on the Belgian market.

Dear colleagues,
 
The Belgian Early Warning System Drugs (BEWSD) was informed about the analysis of an ecstasy tablet containing a high amount of MDMA.
 
It concerns a white oval tablet (see picture), with a “Bugatti” logo which was seized in the region of Antwerp.
 

Especially with regard to the summer dance festivals, the BEWSD would like to warn for these tablets, and for highly dosed MDMA tablets in general.
 
We have also heard reports of highly dosed MDMA tablets with logo “Mitsubishi”, “Domino” and “Superman” (see pictures) circulating on the Belgian summer dance festivals.
The Domino tablets were also reported in a previous EWS alert.
 
  

Also, the presence of 4-fluoro-amphetamine on the summer festivals is suspected (information provided by Breakline, NOT confirmed).

Dear colleagues,
The BEWSD was informed of the death of a 16-year old male. The male died last weekend (April 26-27th) in the hospital after having visited a club in Oostende. On admission, the patient declared “having been dosed by someone”. However, “domino” ecstasy tablets were found on the victim.
It is presumed that the patient died as the result of an MDMA intoxication. The victim presented with all symptoms of a grave serotonin syndrome, and died from complications thereof. At the moment, no toxicology results are available
Also, the presumed MDMA tablets have not been analyzed yet.

Comparable tablets have been found in The Netherlands; analysis of these tablets by the Dutch DIMS system revealed these tablets to contain up to 200 mg MDMA.HCl. However, since the tablets consumed by the victim have not been analyzed yet, there is no certainty concerning the content of the tablets in this specific case.
There is no antidote for an intoxication with MDMA. Treatment is mainly symptomatic. Very important in patients presenting with hyperthermia is careful monitoring and aggressive cooling when necessary, initiated as soon as possible. If hyperthermia isn’t treated swiftly, core temperatures of 42-43°C can be reached, which are usually fatal.

Dear colleagues,
 
The Belgian Early Warning System Drugs (BEWSD) was informed of the analysis of several samples warranting an alert.
The samples were analysed in the framework of the Modus Vivendi drug testing project in Brussels.
 
It concerns the following samples:
 
- Amphetamine powder with a very high concentration of amphetamine (approaching ~100 %)
 
- Ecstacy tablet containing a high amount of MDMA (186 mg MDMA.HCl).
Available tablet data: 9 mm diameter, 5 mm thickness, weight 390 mg. Yellow colour. No further information is available.
 
- A powder sample was found containing 3-methyl-methcathinone (3-MMC), a derivative of the well-known new psychoactive substance mephedrone.
More information with regard to 3-MMC can be found in theattached fact sheet.
 
We would like to ask you to remain vigilant and to contact the BEWSD should you encounter one of these substances (or any other dangerous drugs or combinations).

Geachte collega’s,
 
Het Belgisch Early Warning System Drugs (BEWSD) werd door het Europees Waarnemingscentrum voor Drugs en Drugverslaving (EMCDDA)  op de hoogte gebracht van 18 overlijdens na het gebruik van p-methyl-4-methylaminorex. De overlijdens vonden plaats in Noord-Ierland tussen Juni en December 2013.
 
p-methyl-4-methylaminorex werd ook aangetroffen in tabletten met een “kers”-logo, of een “kruis”-logo (zie afbeelding).
De molecule was ook al betrokken bij de dood van 8 mensen in Hongarije in oktober 2013.
 
Tot op heden werd p-methyl-4-methylaminorex nog NIET aangetroffen in België!
Ook onderstaande tabletten werden voorlopig nog niet in ons land aangetroffen.
 

 
p-methyl-4-methylaminorex is een stimulans, en wordt vooral gebruikt in het uitgaansmilieu, vaak in combinatie met andere drugs (cocaïne, MDMA, …).
Voornaamste (neven)effecten van gebruik zijn agitatie, hyperthermie, coma, schuimbekken, ademhalingsproblemen en hartstilstand.
 
Vermits in bovenvermelde overlijdens tevens andere drugs werden aangetroffen, is het op dit moment nog niet duidelijk welke effecten toe te schrijven zijn aan p-methyl-4-methylaminorex.

Gelieve waakzaam te zijn bij verdachte intoxicaties, en aarzel niet contact op te nemen met het BEWSD indien u verdere vragen heeft.

Le Belgian Early Warning System Drugs (BEWSD) a été informé par l’Observatoire Européen des Drogues et des Toxicomanies (OEDT) de 18 décès après la consommation de la nouvelle substance psychoactive p-methyl-4-methylaminorex.
Les décès ont eu lieu en Irlande du Nord entre juin et décembre 2013.
 
Le p-methyl-4-methylaminorex a été trouvé dans des tablettes avec un logo « cerise » ou « croix » (voir photos en annexe).
La molécule était également la cause de 8 décès en Hongrie en octobre 2013.
 
Jusqu’à présent, le p-methyl-4-methylaminorex n’a pas encore été analysé en Belgique !
Les tablettes ci-dessous n’ont pas encore été trouvées dans notre pays.
 

 
Le p-methyl-4-methylaminorex est une molécule stimulante qui est parfois utilisé dans des milieux festifs, souvent en combinaison avec d’autres drogues (il)licites (cocaïne, MDMA, …).
Les effets indésirables sont l’agitation, l’hyperthermie, le coma, l’hypersalivation, des problèmes de respiration et l’arrêt du cœur.

Geachte collega’s,
 
Het Belgian Early Warning System on Drugs (BEWSD) werd op de hoogte gebracht van de analyse van ‘ecstacy’-tabletten met een ongewoon hoog gehalte aan MDMA.
Deze tabletten werden in de regio Brussel aangetroffen en werden geanalyseerd binnen het Modus Vivendi pill-testing project (tabletten “Mitsubishi” en “Li-Ion”).
Bovendien werden volgende hoog gedoseerde tabletten gerapporteerd door het NICC: “Superman”, “Nintendo” en “Vlinder” (zie bijgaande figuren en tabel).
 
Het betreft tabletten met volgende karakteristieken:

Een overdosering met MDMA kan leiden tot hyperthermie en een serotonine-syndroom (zeker in combinatie met dehydratatie door te weinig drinken en/ of de hitte in het clubmilieu),
en kan zelfs de dood tot gevolg hebben.
 
Indien deze tabletten gevonden worden in jullie regio, aarzel dan niet om contact op te nemen met het BEWSD via bewsd@wiv-isp.be.

Le Belgian Early Warning System on Drugs (BEWSD)a été informé de l’identification de comprimés d’ecstasy avec une composition anormalement élevée en MDMA.
Ces comprimés ont été trouvés dans la région de Bruxelles et ont été analysés dans le cadre du projet de testing del’asbl Modus Vivendi (comprimés « Mitsubishi » et « Li-Ion »).
Egalement, l’INCC nous a informé de l’analyse des tablettes élevées en MDMA (comprimés « Superman », « Nintendo » et « Papillon »).
 
Il s'agit de comprimés avec des caractéristiques suivantes:

Un surdosage de MDMA peut provoquer des symptômes d’hyperthermie et un syndrome sérotoninergique
(surtout en combinaison avec la déshydratation et la chaleur dans des milieux festifs), et peut même provoquer la mort.

Het Belgian Early Warning System Drugs (BEWSD) werd door Apr. Kl-Biol. M. Uytterhoeven (AML Labs) op de hoogte gebracht van de analyse van een poeder,
bevattende MDPV.
Dit poeder werd aangetroffen en voor analyse opgestuurd door de politiezone BODUKAP (Bonheiden, Duffel, Putte, Sint-Katelijne-Waver).
 


MDPV wordt soms gebruikt als vervangmiddel voor cocaïne, en werd in ons land onder controle gebracht in december 2013.
In 2011 werden reeds twee ernstige intoxicaties met deze stof gemeld aan het BEWSD. De stof is vele malen toxischer dan bv. cocaïne.
 
Symptomen van intoxicatie met MDPV zijn vergelijkbaar met deze van een intoxicatie met cocaïne of amfetamine,
d.w.z. de klassieke symptomen van een intoxicatie met stimulantia (verhoogde bloeddruk en hartslag, agressie, palpitaties, angst, paranoia).
In ernstige gevallen kan een psychose optreden.
De behandeling is symptomatisch (diazepam) en ondersteunend.

Gelieve het BEWSD op de hoogte te brengen indien u weet heeft van bijkomende intoxicaties met MDPV, of indien deze stof in uw laboratorium werd aangetroffen.

Le Belgian Early Warning System on Drugs (BEWSD) a été informé par ph. biol-cl. M. Uytterhoeven (AML Labs) de l’analyse d’une poudre contenant du MDPV.
Cette poudre a été achetée en ligne (jukislab.com, prix: ~70€/5 grammes) dans la zone de police BODUKAP (Bonheiden, Duffel, Putte, Sint-Katelijne-Waver).
 
 
 
Le MDPV est parfois utilisé comme produit de substitution de la cocaïne ; la substance est contrôlée dans notre pays depuis décembre 2013.
En 2011, deux intoxications au MDPV ont déjà été rapportées au BEWSD.
 
Une intoxication au MDPV est caractérisée par les symptômes classiques d’intoxication aux stimulants : une pression sanguine élevée,
une augmentation de la fréquence cardiaque, des palpitations, de l’agressivité, de l’angoisse, de la paranoïa.

Le MDPV est beaucoup plus toxique que par exemple la cocaïne.
Dans des cas sérieux des psychoses sont possibles.
Le traitement est symptomatique (diazépam) et de soutien.
